The Numerical Rating Scale (NRS) is a validated tool for quantifying patient-reported pruritus.1, 2

It is based on a simple assessment of pruritus from 0 (no pruritus) to 10 (worst pruritus imaginable).1

The NRS score correlates well with the severity of the disease:1
